When the form appears in your browser, you can:

b) The form(s) can be downloaded, filled out and emailed as an attachment to westwoodanimalhospital@gmail.com 
                       OR
a) Hit the print icon on your browser to print the forms.  They will need to be faxed (913.677.0203) or mailed to us.
                       OR
b) If all else fails, the text can be copied from your browser, inserted into a Word for Windows document, filled out and emailed back to us. 

 Videos can be very helpful for assessment and diagnosis  If you have a video of your pet's behavior and can bring it to the consultation, please discuss this with Dr. Hunthausen.
